Chronic Disease Management Plans
GP Management Plan
If you have a chronic disease you may be eligible for a GP Management Plan. To be eligible, a person must have a chronic disease that is expected to last more than 6 months. Examples of this include diabetes, cancer, heart disease, COPD and chronic kidney disease.
Team Care Arrangement
For patients with a current GPMP you may be eligible for a Team Care Arrangement if you have at least 3 health care providers actively involved in your care. Your GP is included. For example, someone with diabetes may see their GP, an endocrinologist, and a podiatrist for diabetic foot checks.
Medicare offers up to 5 subsidised visits to certain allied health professionals if you have a Team Care Arrangement (Enhanced Primary Care or EPC). These EPC visits can only be used in relation to your chronic disease.
Chronic Disease Management Plans should be reviewed every 3 to 6 months.
